The categories of service users to whom we provide services are:
Caring for a loved one with dementia poses many challenges for families and caregivers. People with dementia conditions such as Alzheimer’s and related diseases have a progressive biological brain disorder that makes it more and more difficult for them to remember things, communicate with others and take care of themselves. In addition, dementia can cause mood swings and even change a person’s personality and behavior. At Essence Telford, our staff are well trained on practical strategies for dealing with the troubling behavior problems and communication difficulties often encountered when caring for a person with dementia.
Independence is something we can take for granted, but for many it’s something to be cherished. At Essence Telford, we provide daily, around the clock at home nursing support enabling anyone who needs qualified care to maintain their independence for longer. Our dedicated carers are able provide you with one to one care, which isn’t possible in residential homes where staff to client ratios are higher. This way you can trust that the care you or your loved one receives is more dedicated and personal than in a care home. We are there 24 hours a day, for people who want to retain an independent lifestyle, but whose age, health or mobility make everyday life more of a challenge.
Our rehabilitation department is committed to assisting patients in reaching their goals. We achieve excellent outcomes by providing personal attention to each patient, addressing functional transfers and mobility issues, practicing “everyday” activities, and providing patient/caregiver education to ensure success after the patient returns home. “Family meetings” are arranged with our therapists along with our social services department to discuss patient’s progress and answer any questions or concerns families may have regarding the patient’s rehabilitative goals.
In the final stages of many terminal illnesses, care priorities tend to shift. Instead of ongoing curative measures, the focus often changes to palliative care for the relief of pain, symptoms, and emotional stress. Ensuring a loved one’s final months, weeks, or days are as good as they can be requires more than just a series of care choices. Anticipating the demands of end-of-life caregiving can help ease the journey from care and grief towards acceptance and healing.
